The Computer Core of the SSRI is pleased to offer internet-based data collection services.

 
The following demonstration has been designed to collect basic information about parents and their children. The system could be modified to collect any other type of information that follows the same format of a record with an unknown and variable number of associated sub-records.

  • Upon entering the site, the respondent provides some inital contact information such as name, address, phone numbers and email address.
  • The system checks for valid entries in the required fields and displays the appropriate error message(s) if necessary.
  • If the respondent has furnished all the required information, it is then displayed on the screen with a form to enter the first child's information.
  • When each child's information has been submitted, it is displayed with the parent's information and the choice to enter another child or complete the data collection process.
  • After all children's data has been entered, the respondent is given an opportunity to review the information they provided and make corrections.
  • An email message is sent to the site administrator, informing him or her that new data has been collected by the system. This email contains a link to the administrator login page.
  • When the administrator successfully enters the UserName and Password, an email message and Microsoft Access attachment are sent to the address on file for that user. The attachment contains all data collected by the system since the previous data transfer.
